At Options for Life Foundation (OFLF), we recognize the financial strain that life-threatening health challenges bring. Our mission is to provide financial assistance to those in greatest need, ensuring access to integrative and complementary treatments for individuals who otherwise cannot afford them. Please review the eligibility criteria below to see how we may assist you. Our resources are limited, and we prioritize applicants with the most significant financial need.

Eligibility Criteria for Financial Assistance:

  • Limited or No Family Support: Applicants must not be receiving substantial financial help from family members who are able to cover major expenses like healthcare, housing, or daily living costs. If family support is provided, detailed financial information will be required to assess the need.

  • Solely Relying on Government Assistance: Priority is given to those solely relying on programs like SSDI, Medicare, Medicaid, or food stamps. We especially consider individuals with limited financial means who do not have significant assets, including properties, savings, or investments that could be used to fund their care.

  • Greatest Financial Need: Our focus is on applicants with substantial financial challenges and no other available resources, including assets like real estate or savings, that could reasonably be used to cover treatment costs.

Documentation Requirements

Please be prepared to provide the following:

  • Acceptance Letter (From the medical facility and treating physician, including treatment plan and costs.)

  • Tax returns (last two years, if applicable)

  • Proof of family financial support (if applicable)

  • Government assistance documentation (SSDI, Medicare, Medicaid, food stamps)

  • Pay stubs or other proof of income, including unemployment (if applicable)

  • Bank statements (last two months)

  • Proof of assets (property, investments, etc.)

  • Business ownership documentation (if applicable)

  • Investments that can be liquidated (e.g., 401k, stocks)

  • Spousal support documentation (if applicable)

  • Child support documentation (if applicable)

  • Inheritance or trust fund distributions (if applicable)

  • Insurance payouts (life or disability insurance)

  • Other recurring disbursements (e.g., annuities, settlements)

  • Charitable donations or support (e.g., GoFundMe)

Application Review Process

Please Allow Up to Three Weeks for Decision

Please note that delays may occur if any required information is missing from your application. If this is the case, we will reach out to request additional details.
